Solution for "not linked to an action" Patient Information
Sometimes patient information appears at the patient level with the message "not linked to an action". This means the information was created at the programme level but not linked to a specific action. As a result, this information is automatically associated with all patients in the programme but is set to not visible by default.
Steps to remove patient information at the programme level:

Open the patient information:
Open the patient’s information in a tab. -
Open the programme:
Open the relevant programme in another tab. -
Add an action:
Within the programme, click Add action at the top right under Actions. -
Use the Action Builder:
The Action Builder will open.
Click Patient information in the menu on the right. -
Remove the patient information:
Select the patient information you want to remove.
Click Delete at the bottom.
